Submit your case

Submit your case.

Send us the records you hold, tell us what you want to know, and pay the fixed fee. We match the case to a senior consultant in the right specialty and the report comes back to you in five to seven working days.

If the records you send cannot support a useful review, we tell you and refund the fee in full.

Do not use this form if the situation is urgent

This form is not monitored around the clock and a review takes days. If something is deteriorating now, contact your local emergency number or the doctor treating you.

Before you upload anything

Your records will be transferred to India, where the reviewing specialist is based. That is an international transfer of health data outside the UK and the EEA, and you are asked to consent to it explicitly at the bottom of this form. What happens to your records.
